Nominations for the 2013 regional awards open on the 6th March 2013

Choose the category or categories for which you want to nominate your colleague or yourself. If you are entering more than one category, separate entry forms must be completed.
Read the judging criteria carefully.
Write an account of no more than 300 words saying why your colleague (or you) should be considered. An additional account from a colleague or service user/family carer is helpful but not essential.
If you would like to send more information to support your online nomination, please send through your testimonial to sasha@care-awards.co.uk. Please make sure that the name of the nominee and the award category is included on all supporting information.Please note that testimonials should be no more than one A4 page.
All candidates will be shortlisted from you submission so please tell us as much as you can, the shortlisting panel will not be able to judge from one sentence.
Nominations for each region will close at 4pm on the closing date – nominations will not be accepted after this time.
Closing dates vary per region please check the specific regional webpage for all details.
