How to enter
Choose the category or categories for which you want to nominate your colleague or yourself. If you are entering more than one category, separate entry forms must be completed.
Read the attached judging criteria carefully.
Write an account of no more than 300 words saying why your colleague (or you) should be considered.
An additional account from a colleague, or service user/family carer is helpful but not essential.
Please make sure that the name of the nominee/entrant and the award category is included on all separate pieces of supporting information.
Complete this entry form and attach it to all other separate pieces of supporting information.
Closing date for entries is 19th April 2010. Post the entry to: The Great British Care Awards, 21 Regent Street, Nottingham NG1 5BS or fax to 0115 959 6148 or email carolyn@care-awards.co.uk.
Rules for Entry
The awards are open to all staff working in the social care sector.
All entries are treated confidentially.
Entries will be judged by a panel of reputable, independent and impartial adjudicators.
Entries will not be returned.
One copy of the submission is required.
Use one entry form per entry. Photocopies are acceptable.
Judging will take place on the 5th May 2010 at the Marriott Hotel, County Hall, London.
Winners of the regional awards 2009/2010 are automatically invited to attend the national care home finals at the Guildhall, London, on 5th June 2010.
There will be up to nine finalists for the national finals in each category representing the West Midlands, East Midlands, North West, North East, Yorkshire and Humberside, London, East of England, South West and South East.
